Facility Use Terms and Conditions
With the submission of each Facility Use Application, the applicant verifies authorization to act as the legal agent of the named organization for the purpose of application for and use of a Florence One Schools facility. The applicant acknowledges receipt and agrees to the conditions of building use as specified here in. The applicant agrees that it shall ensure that use of the facility is in compliance with all applicable Florence One Schools safety regulations.
The applicant recognizes that the school district’s property and general liability insurance policies do not provide coverage to external organizations using school facilities. The applicant is required to provide a current certificate of comprehensive general liability insurance in the amount of $1,000,000 per occurrence. The certificate of liability insurance must have an endorsement naming Florence 1 Schools as an additional insured and must be received not later than 10 days prior to the event. To meet these requirements, consult your insurance carrier or search online for special event insurance carriers.
The undersigned is over 21 years of age and has read this form and agrees to comply with all of the terms of the proposed use. I certify that as the officially-designated officer of the applicant organization, I have the authority to sign all forms and agreements, including this one, and that my signature indicates a full understanding of all the obligations assumed when using school facilities or school grounds. I the undersigned, agree to be responsible to Florence One Schools for the use and care of the facilities. He/She, on behalf of the user organization does hereby covenant and agree to defend, indemnify and hold harmless the district from and against any and all liability, loss, damages, claims, or actions (including cost and Attorney’s fees) for bodily injury and/or property damage, to the extent permissible by law, arising out of or in connection with the actual or proposed use of the district’s property, facilities and/or services by the user organization.
I, the undersigned, have made the above request on behalf of the above listed organization or group; with the understanding that I am to be held directly responsible for all personal injuries and for replacing or repairing any damages, (accidental or vandalism) directly caused by the group or its invitees or participants. It is also understood that the privilege of using the building will be terminated until such damage has been repaired, and/or future requests for use of the facilities may be denied.
The use and possession of illegal drugs, tobacco and/or alcoholic beverages by any person or group on Florence One Schools facilities is strictly prohibited. If there is any evidence of this use or possession, the contract will be voided immediately and future use may be denied.
For safety reasons, all rentals will be required to have security services provided by the Florence City Police or Florence County Sheriff departments, at applicant expense. With a minimum of three (3) hours, officers are required for the duration of the event. Service will be arranged by Florence One Schools.
Applicant is to be out of facility by time listed on contract. Any time overage will result in extra cost that will be billed to applicant for immediate payment. Invoice for overage not paid within 10 days after date of notice will result in a loss of privileges for future rentals.
Florence One Schools reserves the right to cancel permits when such action is in the best interest of the school district. In the event of school closure for weather or emergencies, use of the facility will be automatically cancelled without notice.
