     You will find a printable copy of the official Sports Physical Form for the 2020-2021 school year attached below. This is the only Physical Form that will be accepted for Sneed Athletics. Make sure each section is filled out COMPLETELY.

     

    NOTE: This Physical Form MUST be uploaded to the PlanetHS website before beginning any activity (conditioning, tryouts, etc. For more information on this process, you can go to the PlanetHS homepage, located in Sneed's Athletics Directory.
