We are not allowed to give your child any medication unless we have a signed and dated request from the parent stating the nurse is allowed to give the child the medication and a signed and dated statement from your child’s doctor stating the name of the medicine, dosage, and time it is to be given. The medicine must be in the properly labeled container. All medications should be given to the school nurse. Students are not allowed to medicate themselves at school without special permission from administration and their doctor. The District medication Form can be found on the District Website under Nursing. It is very important that you let the nurse know right away if he/she has a health problem such as allergies, diabetes, poor vision or hearing, epilepsy or any other condition that may require special attention. Please include this on the Emergency Card and tell your child’s teacher.
