A Physician Must Complete the Form Below. The form must be sighed and dated by the physician, parent, and student athlete. 

Be Sure That It Is Dated AFTER April 1st of the previous school of the sport the student plans to participate in. For example: April 1st, 2021 is good for the entire 2021-2022 school year.
